Before your first appointment we will request a medical referral and copies of any relevant reports you may have to assist us in providing informed assessment and/or treatment. For children, NAPLAN results can be most helpful while other school reports are not required.
Before your first appointment we require that you fill in our new client information and consent forms. We require that NDIS participants complete a service agreement form which we will provide to you.
At the conclusion of your first consultation, you will be advised of the assessment or therapeutic approach that we intend to use with your agreement, and address any of your questions.
